How To Qualify For Our Services

Medicare Criteria for Home Health Services

Residence: The patient must receive all home health services at his or her place of residence whether it be temporary or permanent.

Homebound: The patient must meet Medicare’s definition of homebound which states that it is a taxing effort to leave the home and absences from the home are infrequent and of short duration. Patients are allowed to attend all medical appointments, religious services, adult day care activities, and an occasional social outing.

Skilled Care Required: The patient must be in need of one or more of the following services to begin home care. These services include Physical Therapy, Speech Therapy, or Skilled Nursing. The patient’s physician must provide an order for these services. These services qualify a patient to receive other services including Occupational Therapy, Home Health Aide, and Medical Social Services.

Physician’s Care: The patient must be under the care of a physician who certifies and reviews the home health plan of care and the patient’s homebound status.
